Thanks for coming back to me on this.
I disabled WP Super Cache by Automatic – and it instantly fixed the problem. So I’m very sorry for wasting your time with that!
Any ideas what the fix would be if I wanted to continue to use caching?

I would check a couple of things:
1) make sure S&F results URL, is using the correct version of the URL (https for example, not http) – wp super cacche also modifies htaccess, and I suspect, its possibly on the redirect from http -> https (which I think that plugin forces, provided you set the option) that the url value might be double encoded.
2) the only other way I could see super cache doing it, is if they automatically escaped all <form>
input values where method="get"
– in this scenario you would have to see if you could get super cache to leave certain pages / forms out…
