Hi there,
I’ve set up a search form and it is displaying properly on this page: http://yorkshirebus.com but not this page: http://yorkshirebus.com/slide-test/
Any ideas why it’s not displaying correctly?

No idea why but the JS file for search & filter is not being loaded
On the first page you have this script loaded in the source
http://yorkshirebus.com/wp-content/plugins/search-filter-pro/public/assets/js/search-filter-build.js
and on the second this script is completely absent.
Something in your theme or a plugin is unhooking S&F scripts.
