Thanks Trevor. I did as you suggested and resolved my issue. Not surprised that the plugin works when properly configured =)
To summarize, in case others stumble upon this thread:
I removed the duplicate form that I created manually.
I then used the (+) icon to create a translated version of the form.
I then configured the form just like the default language.
