    What plugin and what categories/taxonomies or custom fields (post meta)?

    Not a “famous” plugin: his name is helpie (is a KBase plugin that have his category and tags)

    They will probably be Taxonomies. Add a Taxonomy field to the Form UI, and then look in the drop down list in the settings for them.
