Hi,
I installed the free version and works fine. I then purchased the Pro version and doesn’t work. The submit button is not active.
The site is http://itrsonline.org/wordpress/papers
I tried deactivating all the plugins and still does not work.
Do you know what could be wrong?

I’m a little confused :/ I went the address and all seems ok! Clicking submit takes me to this page:
