I have tested a number of possibilities within the form settings and none of these explain the issue. I suspect, at this stage, that another plugin is conflicting. When it is safe for you to do so, can you deactivate all other plugins and test? If there are some plugins that have to be activated for the page to work, let me know which ones and I will try to make a judgement as to whether they are important to this test or not.
