    Old rule of thumb in WordPress. If you get 404 pages, it may be because the Permalinks are broken. I must recall that more quickly next time. So I opened that Permalinks settings page and re-saved it without changing anything ….

    Ah. These are the instructions to follow for the Shortcode method. Don’t worry about what the results look like. You can have the form in a sidebar as before and the results in the main content if you wish:
