    We’ve had a search & filter set up on a site for over a year. Just last month it mysterioiusly disappeared from the front end and there were no forms in the backend.
    We rebuilt them based on a backup we had.
    Then today we noticed that the form was gone again. No form on the backend either.

    Is there a way to find why this keeps disappearing?

    I’m also now assuming that the plugin updated and this caused the form to be deleted. Is this a common issue?

    There are 3 ways to update. One will delete your forms (#2):

    1. Auto update in WordPress. Forms remain.
    2. In Wp Admin, deactivate the plugin, delete it, install new version. This will delete all forms and cache.
    3. In Wp Admin, deactivate plugin. Use ftp to delete plugin folder, upload new version using ftp, activate plugin in WP Admin. Forms will remain.
