Search form fields are breaking down in a single line

I have purchased and used your search & filter pro version and placed the shortcode in the below page:
http://www.indianelectricity.com/news/searchpage.php
Where the form is coming fine.

But my concern is to display the same form in each and every page just below the header image. For that I have used the same shortcode in Header page, Index page, single page etc. But after that the form fields are breaking down like below:
http://www.indianelectricity.com/news/

How can I resolve this issue, please guide me in this regard.

I am looking at your second link and it appears to be displaying correctly?
