Not working after upgrade

I hesitated to delete the existing plug-in to upgrade, but it would not install anyway so I finally deleted it and manually installed the new version. My search form was then gone but I had a copy of the site elsewhere so I copied all the settings to reproduce my form but it does not function.

Copy site (old version) functions:

Live site (new version) does not function:
https://www.appdetails.com/articles/?_sft_post_tag=packaging

By not function, I mean when I submit the page simply reloads with all articles displayed ignoring any choices made in the form.

I’ve spent more than an hour rechecking every setting, starting over again and again and I simply cannot make it work.

As an aside to your issue, did you delete the plugin from WP Admin (do not do this as it also deletes the forms you made) or by ftp and deleting the folder?
