I would need more information as to what you want to do.
Do you have a form you have designed and want to change its appearance? If so, I would need to see a live web page with the form as it is now, and a mockup of what you want (ideally made in Illustrator or Photoshop – or equivalent – and saved as an image which you can link to here).

I understand what you desire to achieve. The first part is a grid; two columns side by side with a row under both columns. I think that this could be achieved with Custom Javascript, to add additional elements around our form elements, but the coding of this is beyond the scope or our support. Note that V3 of our plugin should allow you to do this much more easily.
As to the sliders looking as you show them. Our slider uses the jQuery UI slider, and that only basic handles. I am not sure what script (if there is one) would be needed to work and look like the examples you show.
