If the form does not appear, you need to add it using a sidebar widget, or by dding a do_shortocde maybe to the template:

    I did with do_shortocde.
archive template and taxonomy template is almost identical.
It’s strange only taxonomy template does not show the form.|
taxonomy option on the form setting is enabled, of course.

I forgot that I changed archive layout from php template to BB template, sorry.

Reason the dropdown wasn’t working is that I changed setting on only one of them.
So set the same setting to both except “enable on taxonomy” solved the problem.

Question, using these identical form affects its index each other?

I am pleased it is sorted thus far. I am not sure what you mean in that last question. If you mean will using these forms like this cause problems in our index/cache, the answer is that it will not be a problem.

The issue of two forms on one Post Type Archive is complex. The warning we give is there because there is a good chance that they will conflict and cause unexpected results. But, sometimes they don’t and we cannot guarantee trouble free use with two such forms. You would need to make extra sure all is OK in your setup.
