    Can that form not use custom fields? Type Blood is a custom field.

    Ah. The reason I suggested custom field is that sharing a taxonomy between post types is not a good thing, and can cause problems when searching. So, if the two post types you have have the same taxonomies to share, this may cause the searches to be unreliable or not work properly.

    Why do they need to be taxonomies?

    OK. I understand you wish to use Country and City as Taxonomies.

    You have Country as a Taxonomy. You must make a new taxonomy for City, so they are separate.
