It is getting very late here. My best guess on the not going to the right element is that something else in the page load sequence is negating this (from the theme, menu or another plugin).
To not show empty ones, Hide Empty in the field needs to be ON, and Auto Count also. I think if you checked the second, bigger form, those settings might not be in place.
Which, when they are correctly set, I wonder no terms are showing, hence my question about the concept label on each post.
The difference is that our plugin does it with an external JavaScript, but adding it in the URL, as you have tried, the browser instead handles it. One has more priority than the other. I think we use JavaScript to avoid any possible conflicts with our filtering.
