    It should not have worked with three Category fields, and I don’t know why it did.

    Can you make a duplicate of the form and leave it as a draft, so you have a record of how it was set?

    Then, in the original form, delete the second two category fields, and in the third, in the advanced settings, make sure it shows all Category terms by setting it to Sync with Settings.

    In the Tags, Categories & Taxonomies section, remove all the includes. Then test the form. If that works, then move forwards, one step at a time, to what you had before.
