With the Post Type Archive method (which you are using), the archive page is always where it re-directs to, which is what is happening.
Cases is a custom page. So you would have to use our Shortcode Display results method, or a posts grid (like the one in WPBakery Page Builder from CodeCanyon, or the free Post Grid plugin. Links here:

I think you have the Search Operator set to AND. Set it to OR as shown.

In YOUR form, what settings have you used for these highlighted in red (this is one of my forms, so ignore what mine is set to)?
3. The title of the page is determined by your theme. It would require that the PHP of the results page is changed to show the search filters, and this would require custom code. This thread discusses how to start doing what you ask:
