I am sorry, it is not possible, as it is custom coded to that version of the TwentyFourteen theme.
Are you able to send me a live link/URL to your search page so I can take a look at what you have so far, and maybe you could detail what is wrong, needs adding or changing?

You have five forms on the page, but should have just one. The five choice types should be each their own custom taxonomy, split out from the one custom taxonomy that you are currently using (portfolio_entries
).
With the addition of CSS, that single form can then be made to look how you want it.
