Yes, and the form should work fine on them. This is why I suggested using Skype, we can avoid the inevitable delays and misunderstandings common in emails and forum postings.
You want different forms on different taxonomies? By ‘form’ I mean the collection of fields and other controls.
With regard to the URL query strings, can you make sure these two are checked (ON)?

The more I think about this, the more I think that the product CPT must be set up wrong somehow.
Is there a reason why you didn’t use CPT-UI plugin to make them?
