I have made 3 changes to the form (4842) to see if this helps?
#1 Enabled Auto Count (both settings)
#2 Enable Hide Empty (otherwise if you show an empty category and the user selects this ….)
#3 Show count (which enabled me to see that at least one category – uncategorised (it was last) – is empty)2 and 3 I did only for category, but hide empty should really be on for all fields.
You may want to switch off show count.
